Frequently schools or sports associations require yearly physicals prior to participation in sports.  At Premier Healthcare, we provide this service for all patients.  Insurance does not reimburse for sports physicals, so we offer these services for a small fee of $25.

Do I need to bring anything?

 

  All schools and sports associations will provide you with the needed forms that will have to be filled out by the athlete and by the provider that is conducting the physical. Nothing else needed for the sports physical.
